Anxiety therapy in San Antonio

Worry is useful right up until it is not. When it starts making your decisions, costing you sleep, and quietly removing things from your life, it has stopped being a personality trait and become something treatable.

What anxiety is doing

Anxiety is a threat-detection system doing its job with the sensitivity turned too high. It is not irrationality — the machinery is working exactly as designed, it is simply firing at things that are not dangerous. That is why being told to calm down has never once worked on anybody.

The trap is that avoidance works brilliantly in the short term. Every time you cancel the thing, the relief is immediate, and your brain learns that cancelling was what kept you safe. Do that for a few years and the world gets measurably smaller without any single decision to make it so.

The forms it takes

  • Generalised anxiety — worry that moves from topic to topic, most days, for months.
  • Panic — sudden surges of intense fear with strong physical symptoms, often with a fear of the next one attached.
  • Social anxiety — fear of being judged, which is not shyness and does not improve by being made to do more of it unprepared.
  • Health anxiety — where checking and reassurance-seeking keep the fear alive rather than settling it.
  • Anxiety sitting on top of trauma — common, frequently missed, and treated differently.

How it is treated

Cognitive behavioural approaches have the strongest evidence for anxiety disorders. In practice that means understanding the loop you are caught in, examining the predictions your anxiety makes, and then — carefully, at your pace — testing them rather than arguing with them. Doing the thing is what updates the system; thinking about the thing does not.

We also work on the physical end: breathing, sleep, caffeine, and what your body is doing at three in the morning.

What changes, and what does not

The goal is not a life without anxiety — you would not want one, and it is not available. It is that anxiety stops being the thing that decides what you do. Most people notice the change first in something small they had quietly given up.

Questions people ask

How do I know if it is anxiety or something medical?

Get it checked. Thyroid problems, heart conditions, some medications and caffeine all produce symptoms that look like anxiety. A physician ruling those out is a sensible first step and I will ask whether you have done it.

What is the difference between anxiety and a panic attack?

Anxiety generally builds and lingers. A panic attack is a sudden surge that peaks within minutes, with strong physical symptoms, and it is self-limiting — frightening but not dangerous in itself. They are treated somewhat differently.

Can anxiety therapy be done by video?

Yes. Anxiety work adapts well to telehealth, and I see clients anywhere in Texas. Occasionally in-person exposure work is worth doing in the room, and we would discuss that.
